Elderly wheezing: what to know

Understanding the sound
Wheezing is usually a high-pitched sound, often clearer when breathing out, produced as air moves through narrowed airways. Asthma and COPD can cause recurring episodes, while Acute Bronchitis may follow a respiratory infection. Allergic Rhinitis can accompany upper-airway symptoms but does not explain every chest sound. Bronchiolitis is mainly an illness of young children; a clinician should consider other causes when an older adult wheezes. New wheezing can also occur with fluid-related heart problems or a medicine reaction.
The timing matters: note whether it began suddenly, follows exertion, appears at night, or comes with cough, fever, chest tightness or ankle swelling. A clinician may listen to the chest, check oxygen levels and review medicines.
What to do now
Sit upright, rest and move away from smoke, dust or a known irritant. Use a prescribed reliever inhaler exactly as directed and check its technique with a clinician or pharmacist. Do not borrow another person’s inhaler or delay care while trying home remedies. Record triggers and how long each episode lasts, and arrange review for first-time, recurrent or worsening wheezing.
Urgent warning signs
Call emergency services for severe breathlessness, blue or grey lips, inability to speak in full sentences, collapse, confusion, or rapidly worsening symptoms. Sudden wheezing with facial swelling, hives or throat tightness may indicate Anaphylaxis and needs immediate help. Seek prompt medical care for chest pain, coughing blood, high fever, or breathlessness that is new or not settling.
