Understanding mid-cycle pain

Ovulation pain, also called mittelschmerz, is discomfort linked with the release of an egg. It may feel like a cramp or a sharper ache on one side of the lower abdomen and can last from a short time to a couple of days. Tracking the timing, side, intensity, bleeding, and other symptoms can help a clinician compare it with your cycle. Pain that is new, steadily worsening, or unrelated to your usual cycle pattern may have another cause, such as endometriosis or an ovarian cyst.

What may help

Use a warm bath or heat pack wrapped in cloth over the lower abdomen, and rest if movement makes the discomfort worse. Gentle activity may be comfortable for some people. Ask a pharmacist or clinician which pain reliever is suitable if you are pregnant, trying to conceive, have stomach or kidney problems, or take other medicines. Record episodes in a cycle diary, including whether pain affects sex, work, sleep, or exercise. Hormonal contraception can prevent ovulation for some people, but it needs an individual discussion.

Seek urgent help for sudden severe pain, fainting, shoulder-tip pain, heavy vaginal bleeding, fever, repeated vomiting, or a rigid or very tender abdomen. Arrange medical review for pain that lasts beyond your expected mid-cycle window, returns frequently, interferes with daily life, or comes with pain during sex, bowel changes, or difficulty becoming pregnant. These patterns should be assessed rather than attributed to ovulation alone.
