What dyspareunia means
Dyspareunia is pain during or around sexual penetration. It may feel burning, stinging, aching, or deep pelvic pain, and it can begin before penetration, with contact, or afterward. Pain during sex is a symptom rather than a diagnosis, so a clinician will ask about timing, pain location, vaginal dryness, bleeding, discharge, skin changes, periods, childbirth, menopause, medicines, and previous pelvic conditions.
Possible causes include dryness or delicate vaginal tissue related to menopause, irritation or infection, involuntary tightening of pelvic-floor muscles, vulvodynia, endometriosis, pelvic inflammatory disease, or skin conditions such as lichen sclerosus. Endometriosis or an ovarian endometrioma can contribute to deep pelvic pain, but pain during sex does not by itself identify any one condition.
How treatment may help
A clinician may examine the vulva and vagina, check for tenderness, and recommend tests when infection or another pelvic condition is possible. Treatment can include addressing an infection, using a suitable lubricant, treating vaginal tissue changes, or changing an irritating product or medicine. Pelvic-floor physiotherapy may help when muscles are tense or painful; counselling or sex therapy can support comfort, communication, and the fear–tension–pain cycle.
Do not force penetration through pain. Pause, choose activities that do not hurt, and record when the pain occurs and what makes it better or worse. If endometriosis is found, ask whether medical treatment or laparoscopic excision of endometriosis is appropriate for your symptoms and findings.
When to seek care
Arrange a medical assessment for new, recurring, or worsening pain, especially if it affects intimacy or continues after sex. Seek prompt care for severe pelvic pain, fever, unusual or foul-smelling discharge, sores or marked swelling, bleeding after sex or between periods, pain when urinating, or possible pregnancy with pelvic pain or bleeding. These features can need examination and treatment rather than self-care alone.
