Double mastectomy for men: what to know

A surgeon removes breast tissue from both sides and may assess nearby lymph nodes. The incision pattern, nipple management and amount of skin removed depend on the tumour, previous procedures, chest shape and planned treatment. Some men choose a flat chest closure; others discuss reconstruction or a delayed option. A mastectomy treats breast tissue but does not by itself determine whether medicines or radiotherapy are needed.

Breast cancer in men is uncommon, but a diagnosis should be assessed by a team familiar with breast cancer care. Paget disease of the breast can affect the nipple and surrounding skin and may require a different surgical discussion. Loss of sensation, tightness and visible scars can affect body image and intimacy; support is part of recovery, not an afterthought.

Ask about genetic assessment when relevant, lymph-node surgery, drains, pain control and the likely time away from work. Arrange practical support for bathing, dressing and driving. Keep the incision clean and dry as instructed, measure or observe drain output if requested, and avoid lifting until cleared. Gentle arm movement often forms part of rehabilitation, but the correct pace depends on the operation.

A well-fitting soft garment may improve comfort after the team approves it. If arm heaviness or swelling follows node treatment, ask about lymphoedema therapy. Emotional support, peer groups and counselling can help with a changed chest and uncertainty about treatment.

Call the surgical team for fever, spreading redness, warmth, wound opening, new drainage, a drain that stops unexpectedly, or pain that becomes harder to control. Get urgent medical help for chest pain, sudden breathlessness, collapse, or rapidly expanding swelling. Do not wait for a routine appointment if a wound change is progressing.

Ask whether both sides need removal, which lymph nodes will be assessed, and how pathology will guide further treatment. Discuss flat closure versus reconstruction, nipple and sensation expectations, drain care, arm exercises, work restrictions and the plan for follow-up.
