High glucose can make the kidneys produce more urine, creating urgency and increasing the chance of leakage. Over time, nerve changes may affect how the bladder stores or empties urine. Infection, pelvic-floor weakness, prostate problems, constipation, pregnancy, neurological disease, and some medicines can cause similar symptoms. Burning urine, an unusual smell, pelvic discomfort, or blood may point to another problem that needs review.

Arrange an appointment if leakage is new, frequent, or interfering with daily activities. Note when it happens, whether coughing or urgency triggers it, how often you pass urine, and any thirst or burning. Keep fluids balanced rather than severely restricting them, and avoid bladder irritants if they clearly worsen urgency. Continue prescribed diabetes medicines and ask about pelvic-floor exercises or bladder training.

Seek urgent care for inability to pass urine, severe lower-abdominal pain, blood in the urine, fever with back or side pain, or new leakage with leg weakness, numbness around the groin, or loss of bowel control. Prompt assessment is also needed for vomiting, confusion, or severe thirst and urination with weakness.
