Why low blood sugar can feel like weakness

A drop in glucose can leave your arms and legs feeling heavy, shaky or lacking strength. You might also sweat, feel hungry, dizzy, anxious or unable to concentrate. Weakness may improve as glucose returns to a safe level, but the symptom alone cannot establish its cause.

Other problems, including dehydration, infection, anaemia, medication effects or a neurological condition, can also cause generalised weakness. Sudden weakness on one side, facial drooping or difficulty speaking is an emergency rather than a symptom to monitor at home.

Steps to take

If you have a glucose meter, check your reading when symptoms begin and follow the treatment plan given by your diabetes team. If you are awake and can swallow, take the recommended quick-acting glucose, then reassess as directed. Once you feel better, have the planned meal or snack if your clinician has advised this.

Avoid driving or physical activity until the weakness has settled. Note the timing, recent food, exercise, alcohol, medicines and glucose reading. This record can help identify patterns and guide safer treatment changes.

When to get medical help

Seek emergency help for fainting, seizure, severe confusion, inability to swallow, one-sided weakness, speech trouble, chest pain or breathing difficulty. If the person is drowsy or unconscious, do not give food or drink by mouth.

Contact a clinician soon if weakness keeps returning, lasts after glucose has improved, occurs without a clear trigger, or begins after a medicine change. Repeated low readings may require a review of meals, activity and treatment.
