Does bronchiolitis cause wheezing?

Bronchiolitis often starts with a runny nose and cough before breathing becomes noisier. Inflamed small airways narrow as the child breathes, producing a musical or whistling sound. Fast breathing, chest pulling in under the ribs, tiring during feeds, and irritability may occur as well. The sound can change as mucus moves. Wheezing alone cannot identify bronchiolitis, because other airway conditions can sound similar.

Keep the child calm and offer smaller feeds more frequently if breathing makes feeding difficult. Hold the child upright while awake and clear nasal secretions gently. Keep smoke, strong fragrances, and other airway irritants away. Do not use someone else’s inhaler or start antibiotics without medical advice. Observe the child rather than relying only on the loudness of the wheeze.

Get urgent help for severe breathing effort, pauses, blue or grey lips, limpness, or inability to feed. Seek prompt assessment if the ribs or neck pull in with each breath, breathing is steadily faster, the child is unusually sleepy, or wet nappies decrease. A clinician should assess any child whose breathing is worsening, even if the wheeze later becomes quieter.
