Difference between cramps and ovulation pain

Menstrual cramps are caused by the uterus contracting and often feel like aching or cramping across the lower abdomen, with back discomfort or nausea. Ovulation pain, also called mittelschmerz, can be a one-sided twinge, ache or sharper sensation near the middle of the cycle. It may switch sides in different cycles and often settles without treatment. Pain that is severe, lasts beyond the expected window, or interferes with daily activity deserves a broader assessment; endometriosis and ovarian cysts are examples of conditions that can cause pelvic pain.

Note the first day of bleeding, the day pain starts, its side, intensity, duration and accompanying symptoms. Gentle movement, heat on the lower abdomen and rest may help. Use pain medicine only as directed and check suitability if you are pregnant, trying to conceive, have stomach or kidney problems, or take other medicines. A clinician can discuss options when pain repeatedly disrupts work, sleep or sex.

Seek urgent care for sudden severe pelvic pain, fainting, fever, repeated vomiting, heavy bleeding, a rigid or swollen abdomen, or pain during a possible pregnancy. Arrange a medical review for new pain, pain that worsens over cycles, pain between periods or pain during sex. Urgency is guided by these warning features, not by whether pain occurs near ovulation.
