What should you know about dermatitis on one hand only?

One-sided symptoms may follow contact with a tool, chemical, glove, phone, sports equipment or another object used mainly by that hand. Hand eczema can also begin asymmetrically. A fungal infection may cause scaling, a defined edge or changes around the nails and can be mistaken for eczema. Cracked skin, itch, redness and thickening help a clinician judge the pattern, but a photograph or visual description cannot replace an examination.

Wash with lukewarm water and a gentle fragrance-free cleanser, then pat dry. Apply emollient therapy often, especially after washing, and cover painful cracks with a clean dressing. Avoid suspected chemicals and do not share towels. A topical corticosteroid may help eczema but can worsen an undiagnosed fungal infection, so use one only when advised. Persistent hand eczema may be considered for narrowband UVB phototherapy by a specialist.

Book an appointment if the one-sided rash is spreading, repeatedly cracking, involving the nails, or failing to improve after irritants are removed. Seek prompt care for pus, increasing warmth, swelling, severe pain, red streaks or fever. A clinician may examine skin from the affected area or nail before choosing treatment.

Ask whether the pattern suggests eczema, contact irritation or fungal infection, and whether testing is needed. Discuss which emollient to use, whether a steroid is appropriate, and what workplace or household exposures to change.
