Deep dyspareunia symptoms: what they can mean and when to seek care

People describe pressure, a sharp twinge, cramping, burning, or soreness behind the pubic bone or deeper in the pelvis. It may happen only with deeper thrusting, in certain positions, around menstruation, or after sex rather than during it.

Several conditions can present with deep pain during sex. Endometriosis may cause pelvic pain that follows a menstrual pattern. An ovarian endometrioma can be associated with pelvic discomfort, while pelvic inflammatory disease may occur with unusual discharge, fever, or pain outside sexual activity. Genitourinary Syndrome of Menopause can make vaginal and surrounding tissues drier and more sensitive. The symptom alone cannot identify its cause.

Pause or change any activity that causes pain. Talk openly with your partner, allow more time for arousal, and try positions where you control depth and movement. A water-based lubricant may reduce friction, but it will not treat pain arising deeper in the pelvis.

Keep a brief record of timing, location, intensity, menstrual cycle, position, bleeding, discharge, urinary symptoms, and bowel symptoms. A clinician may use this history, an examination, pregnancy testing or infection testing when relevant, and imaging when indicated to narrow the possibilities.

Arrange a gynaecology or primary-care appointment for pain that keeps returning, limits intimacy, or continues after sex. Seek prompt care for severe or rapidly worsening pelvic pain, fever, fainting, vomiting, heavy bleeding, or pain with a possible pregnancy. New bleeding after sex, foul-smelling discharge, or pain with urination also warrants timely assessment.

Ask: Could the location and timing suggest endometriosis, an ovarian endometrioma, infection, or tissue changes related to menopause? What examination or tests fit my symptoms? Which pain-relief options are suitable, and when should I return if the symptoms continue?
