Understanding DCIS and LCIS found together

DCIS is contained within milk ducts. Classic LCIS involves milk-producing lobules and is generally treated as a marker of increased future breast cancer risk rather than as an invasive cancer. The exact LCIS subtype matters because non-classic forms can affect surgical planning.

Both conditions are non-invasive, yet they contribute different information. DCIS usually identifies the area that needs local treatment. LCIS can be an incidental microscopic finding and may influence risk discussions for both breasts. The pathologist should specify whether the LCIS is classic or a non-classic variant and whether either finding reaches a surgical margin. Imaging and pathology must be matched so the sampled tissue explains the mammogram abnormality.

Have a breast surgeon review the biopsy, mammogram and pathology wording together. For the DCIS component, options may include lumpectomy with consideration of radiotherapy or mastectomy when breast conservation is unsuitable. The LCIS component may prompt a broader conversation about future imaging, breast awareness and risk-reducing medicine, particularly when hormone receptors and personal risk factors support that discussion.

If the report only says LCIS without naming the subtype, request clarification. A second pathology opinion can be useful when the terminology is uncertain or when it would alter the extent of surgery.

Arrange clinical assessment for a new lump, bloody nipple discharge, a newly pulled-in nipple, skin dimpling or a persistent change in breast shape. After a procedure, call the care team promptly about fever, spreading warmth or redness, increasing swelling, discharge from the wound or escalating pain. DCIS and LCIS can exist without symptoms, so scheduled imaging remains useful even when the breasts feel unchanged.

You may want to ask: Is the LCIS classic or non-classic? Which finding corresponds to the imaging change? Are the DCIS margins clear? Does LCIS at a margin change the proposed operation? How will follow-up cover both breasts, and should hormone therapy be considered for the DCIS, future risk, or both?
