What HER2-positive means in DCIS

HER2 is a marker involved in cell growth. For DCIS, the boundary of the cells within the ducts remains the defining feature, while the full pathology and imaging findings guide treatment planning.

A pathologist evaluates biopsy or surgical tissue to distinguish DCIS from invasive breast cancer. The report can also include grade, hormone receptors, the amount of tissue involved and, after surgery, whether the edges are clear of DCIS. HER2 may be reported, but its role in routine DCIS decisions is different from its role when invasive disease is present. A positive marker should therefore be read in the context of the entire report.

Request a clear explanation of whether the sample contains DCIS only or any invasive focus. Local treatment is generally planned around the area involved: lumpectomy may be paired with radiotherapy, while mastectomy can be considered when disease is widespread within the breast or clear margins would be difficult. Hormone-receptor status, rather than HER2 status, is the relevant marker when discussing hormone therapy for DCIS.

Do not assume that a HER2-positive line automatically calls for medicines used for HER2-positive invasive breast cancer. Ask the breast specialist which finding supports each proposed treatment.

Report a newly felt breast or underarm lump, bloody nipple discharge, skin pulling, nipple inversion or a persistent breast change without waiting for the next routine visit. Following biopsy or surgery, seek prompt clinical advice for fever, spreading redness, marked swelling, wound drainage or pain that is getting worse. Urgency comes from the symptom or postoperative change, not from HER2 status itself.

Consider asking: Was HER2 tested on biopsy tissue or surgical tissue? Is there any invasive cancer in the sample? What are the grade, hormone-receptor results and estimated extent? Which findings determine my surgery choice? If medicine is suggested, what feature is it intended to treat?
