Dark urine and brown discharge: what to know

Brown discharge commonly represents older blood leaving the vagina, including around the beginning or end of a period. It may appear on underwear or tissue independently of urination. Dark urine is seen in the urine stream or a fresh container and may be amber, brown, or tea-coloured. Looking at each separately can prevent one colour from being mistaken for the other.

Concentrated urine can deepen after sweating, reduced fluid intake, or a long interval without urinating. Brown urine that continues, especially with yellow skin or eyes, pale stools, itching, nausea, or unusual tiredness, needs medical review. Pelvic pain, unpleasant discharge, fever, or pain when urinating also changes how urgently the symptoms should be assessed.

Use a clean pad or tissue to observe discharge, and if safe, collect a clean-catch urine sample in a clear container before it contacts vaginal blood. Note the cycle day, smell, amount, pain, fever, and whether the colour changes after regular fluids. Do not douche or insert products to clear the discharge. A clinician can examine the discharge and arrange urine testing when the source is uncertain.

Seek urgent care for severe pelvic or side pain, fainting, heavy bleeding, confusion, repeated vomiting, or inability to pass urine. Prompt review is appropriate for fever, pregnancy or possible pregnancy with pain or bleeding, yellowing of the eyes or skin, or discharge with a strong unpleasant smell. Persistent brown discharge or dark urine should not be attributed to the menstrual cycle without checking the pattern.

Ask whether the colour appears to come from urine, the vagina, or both; whether urine or swab testing is needed; and whether medicines, recent sex, pregnancy possibility, or cycle changes affect the assessment. Bring the timing and appearance you recorded.
