Dark Bleeding After Sex: What It Can Mean and When to Get Help

Blood can look brown, rust-coloured, or dark red when it has taken longer to leave the vagina. A small amount may follow friction, dryness, a minor tear, or contact with a sensitive cervix. Cervical inflammation can also cause bleeding after sex; Chlamydia is one possible infection to discuss if there is unusual discharge, pelvic discomfort, pain when urinating, or a new sexual partner.

After menopause, thinner and less flexible vaginal tissue related to Genitourinary Syndrome of Menopause may bleed with contact. Cervical Dysplasia and Cervical Cancer can present with bleeding after sex, so ongoing, recurrent, or unexplained bleeding should be checked even when there is no pain. Vaginal Cancer can also present with abnormal vaginal bleeding, although an examination is needed to identify the source. These possibilities do not mean that dark bleeding itself confirms a serious condition.

Note when the bleeding began, its colour and amount, whether it happens after every episode of sex, and whether it relates to your menstrual cycle. Pause penetration if it causes pain or fresh bleeding, and avoid inserting products until the cause is clearer. Arrange an appointment with a gynaecologist or primary-care clinician for recurrent, first-time unexplained, or postmenopausal bleeding. They may ask about pregnancy possibility, contraception, periods, infections, medicines, and screening, then examine the vagina and cervix and offer tests when appropriate.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or one-sided pelvic pain, fainting, marked dizziness, fever, or feeling very unwell. If pregnancy is possible, bleeding with pain, shoulder-tip pain, weakness, or faintness needs prompt assessment because an ectopic pregnancy can be an emergency. Do not wait for a routine appointment when symptoms are sudden or worsening.

Ask: Could the blood be coming from the vagina, cervix, or uterus? Do I need an infection test or cervical screening? Could dryness or menopause-related tissue changes be contributing? Which symptoms should make me seek urgent help? If a cervical change needs treatment, ask whether a Cone Biopsy is relevant and what the results would clarify.
