Dandruff spots on the scalp: what they may mean

Dandruff can look like scattered white or yellow flakes, sometimes over mildly red skin. Seborrheic dermatitis may produce greasier scale and more persistent inflammation, including around the ears or face. Well-defined thick plaques may fit psoriasis, while small tender bumps can follow follicle irritation. A circular scaly patch with broken hairs, marked tenderness, or spreading borders needs assessment for infection. A spot is a description, not a diagnosis, so note its colour, texture, symptoms, and change over time.

Take a clear photograph in good light if the spot changes, and note recent hair products, dye, sweating, shaving, or friction. Wash gently and avoid peeling scale away with nails or comb teeth. A suitable medicated shampoo can be used as directed when the skin is intact. If the spots remain, multiply, or recur, a clinician may recommend medicated shampoo treatment or topical antifungal treatment after examining the scalp. Avoid sharing combs or hats while an infectious cause has not been excluded.

Seek medical advice for spots that are painful, warm, swollen, draining, heavily crusted, or associated with patchy hair loss. Prompt care is needed for rapidly spreading redness, fever, facial swelling, or feeling unwell. Persistent spots that do not improve with gentle care should be checked because treatment differs between dandruff, dermatitis, psoriasis, and infection.

Ask what the spot most resembles, whether the hair follicles or surrounding skin are involved, and which treatment is safe near broken skin. Show photographs of earlier changes and list all products used recently.
