What should you know about dandruff ringworm?

Dandruff commonly produces loose or greasy flakes with variable itching and usually stays on the surface of the scalp. Scalp ringworm is a contagious fungal infection that can cause scaly patches, broken hairs, tenderness, swollen areas, or patchy hair loss. The word ringworm does not mean a worm is present. Similar-looking scale can also occur with seborrheic dermatitis, psoriasis, or irritation from a hair product, so appearance alone cannot confirm the cause. Itchy scalp and scalp flaking are symptoms shared by several conditions.

Do not share combs, brushes, hats, towels, or pillow coverings if scalp ringworm is possible, and avoid scratching affected areas. A medicated shampoo may reduce shedding of fungal material but may not be enough on its own for an established infection. A clinician may recommend treatment that reaches the hair follicles, sometimes alongside a topical antifungal treatment. For ordinary dandruff, a suitable medicated shampoo and regular washing may be enough. Do not use leftover steroid or antifungal products without advice, because the wrong treatment can blur the pattern.

Book a prompt medical review for patchy hair loss, broken hairs, pus, tender swelling, thick crusts, or a scalp rash that is spreading. Assessment is especially important when a child has these changes or when someone close to you has a confirmed fungal scalp infection. Seek urgent help for rapidly increasing swelling, severe pain, or feeling unwell.

Ask whether the pattern looks more like dandruff, seborrheic dermatitis, or a fungal infection, and whether scalp or hair testing is needed. Clarify whether household contacts need advice, how to clean shared hair items, and when it is safe to resume close contact.
