Pay attention to whether the baby seems itchy or unsettled, whether the scale is spreading, and whether there is oozing, crusting, swelling, or a strong smell. Hair should not be pulled away from attached scale.

Wash with a mild baby cleanser, rinse well, and pat dry. A soft brush can loosen scale only when it is already coming away easily. Never scratch, pick, or force off crusts. Avoid essential oils, adult dandruff products, fragranced lotions, and medicated treatments unless the baby’s clinician recommends them.

If scale is thick or persistent, ask a paediatric clinician about a suitable softening product and washing routine. Keep products away from the eyes and stop any product that causes redness or swelling.

Contact a clinician if the scalp becomes very red, swollen, painful, wet, bleeding, foul-smelling, or covered with spreading crusts. Prompt assessment is also needed if the baby has fever, seems unwell, feeds poorly, or develops hair loss or a rash elsewhere. Babies with extensive or rapidly changing scalp changes should be examined rather than treated with adult products.

Ask whether the pattern looks like cradle cap, eczema, irritation, or infection, and which cleanser or softening method is safe for your baby’s age. Show the clinician every product used on the scalp.
