Dandruff and psoriasis: how to tell them apart

Dandruff often produces loose white or yellowish flakes on the scalp. Psoriasis can create thicker, well-defined areas with a silvery scale and may extend beyond the hairline. Psoriasis may also appear on elbows, knees, nails or other skin, with dry skin, redness, cracking or peeling. The pattern is more useful than the amount of flaking alone.

Scratching either condition can worsen irritation and create small cracks. A scalp rash is not automatically psoriasis; contact irritation, eczema and other problems can look similar. A clinician can inspect the scalp and ask about skin and nail changes.

Use a gentle cleanser, lukewarm water and fingertips. Do not pick at attached scale, because lifting it forcefully can injure the skin. Keep a note of affected areas, triggers and whether scale returns quickly after washing. If psoriasis is suspected, arrange dermatology advice rather than relying on repeated dandruff products; treatment may need to soften scale and calm inflammation. Follow directions carefully when applying any scalp medicine.

Seek care for thick plaques, bleeding cracks, painful areas, spreading redness, pus, or new patchy hair loss. Book an appointment if you have persistent skin scaling despite careful care, or if similar changes affect nails, ears or other body areas. Urgent help is needed for facial swelling or breathing trouble after a product.

Ask whether the pattern fits dandruff, psoriasis or another rash, which areas should be treated, and how to protect the scalp while scale loosens. Mention joint stiffness or nail changes if present.
