What should you know about cyst under scalp?

Pilar cysts arise from structures associated with hair follicles and commonly feel like firm, rounded nodules beneath the scalp. The overlying skin may look normal. A lump can become tender after pressure from a comb, helmet, pillow, or hairstyle, and inflammation can cause redness or drainage. Other conditions can also produce a scalp lump, so a clinician considers its firmness, mobility, surface, growth pattern, and symptoms.

A scalp cyst is not automatically a reason for treatment. Removal may be considered when it is painful, repeatedly inflamed, catches during grooming, becomes difficult to monitor, or causes ongoing concern. The clinician may inspect the area closely and decide whether a procedure or observation is more appropriate.

Do not pop, cut, or repeatedly press the lump. Wash the scalp gently and avoid picking at a crust or opening. Keep hair tools and pillowcases clean, and avoid tight styles that pull over the swelling. During an appointment, mention how long it has been present, whether close relatives have similar lumps, and any previous episodes of swelling or discharge.

If removal is chosen, a clinician usually numbs the area, makes a controlled opening, and aims to take out the cyst and its capsule. Follow instructions about washing, dressings, activity, and signs of infection after the procedure. Hair may need to be moved or trimmed around the treatment area, depending on its location.

Get prompt care for rapidly increasing pain or swelling, spreading redness, pus, fever, or feeling unwell. Arrange a medical review if the lump is hard, fixed, enlarging, bleeding, ulcerated, or newly associated with persistent headaches or neurological symptoms. These signs can have several explanations, but they warrant examination rather than home drainage.
