What should you know about a cyst ruptured under the skin?

When the cyst wall breaks, its contents can spread into the surrounding tissue rather than leaving through a small surface opening. This may trigger a firm, painful or red area, even when there is no clear cut on the skin. The inflammation can resemble an abscess, and a clinician may need to distinguish the two. A ruptured cyst may also leave its lining behind, allowing a lump to return.

Do not squeeze, lance or massage the area. Keep the skin clean, reduce friction and use a clean covering if there is drainage. Note when the change began and whether the redness or swelling is spreading. Depending on the examination, treatment may include an intralesional steroid injection, abscess drainage or removal of the cyst. Do not start leftover antibiotics without medical advice.

Seek prompt care for rapidly expanding redness, increasing warmth, severe or escalating pain, pus-like drainage, fever, chills or red streaks. Urgent assessment is appropriate if swelling affects the eye, mouth, breathing or swallowing. A persistent lump after the pain settles should still be checked because rupture does not confirm that the cyst lining has gone.
