Why cramps happen
A cramp is a sudden, involuntary tightening of a muscle, often in the calf, foot or thigh. It may follow strenuous exercise, prolonged sitting, dehydration or a muscle strain. Medicines, circulation problems and changes in mineral or hormone balance can also contribute. Chronic Kidney Disease, Hypothyroidism, Peripheral Arterial Disease and Vitamin D Deficiency are examples a clinician may consider when cramps are persistent or accompanied by other symptoms.
Relief and prevention
Stop the activity, gently stretch the affected muscle and massage it until the tightening eases. For a calf cramp, straighten the leg and draw the toes towards you if this is comfortable. Use warmth for tightness or a cool pack after strenuous activity, with a cloth between the skin and pack. Drink fluids regularly, warm up before exercise and increase activity gradually. Do not start mineral supplements without medical advice.
When to seek care
Book a review if cramps are frequent, severe, disrupt sleep, last longer than expected, follow a new medicine or occur with weakness, numbness or swelling. Seek urgent help for a painful swollen leg, a cold or pale foot, severe weakness, dark urine after strenuous exercise or cramps with confusion and inability to keep fluids down. These features need prompt assessment rather than home treatment alone.
Questions for your doctor
Ask whether your medicines, hydration, circulation, thyroid function or vitamin status could be involved. Bring a record of the muscle affected, activity, time of day, duration, associated symptoms and what relieves the cramp.
