Croup is swelling around the voice box and upper windpipe, often during a viral illness. The familiar barking cough may be absent early on, mild, or not noticed. A clinician may need to check whether the sound is stridor, wheezing, congestion, or another cause of noisy breathing.

What croup without cough can mean

A child with croup may have a hoarse or husky voice, a harsh high-pitched sound when breathing in, a runny nose, fever, or symptoms that become more noticeable at night. The cough can develop later, remain subtle, or be missing from the early picture.

Noisy breathing is not specific to croup. Wheezing can occur with asthma or bronchiolitis, while nasal blockage can make breathing sound different. Seek emergency help immediately if the child is struggling to breathe, the skin between or under the ribs is pulling in, the lips look blue or grey, the child cannot speak or cry normally, is drooling or unable to swallow, becomes unusually sleepy, or has pauses in breathing. Keep the child upright and calm while help is arranged.

A harsh breathing sound that continues when the child is calm or resting deserves urgent assessment, even when there is no cough. Sudden noisy breathing after choking, a sting, food, or medicine exposure also needs emergency care.
