Croup voice changes: hoarseness and noisy breathing

The larynx, or voice box, sits above the windpipe. Viral swelling there can change the sound of crying, talking, or coughing. A child may sound husky, raspy, or unusually quiet. The classic cough is harsh and barking. Stridor is different: it is a high-pitched noise made mainly when breathing in and may signal narrowing of the airway. Voice hoarseness alone does not show how severe the illness is, so listen for breathing effort and observe the child when calm.

Keep the child upright, close, and calm. Speak softly and avoid activities that trigger crying or shouting. Offer frequent sips if swallowing is easy, and keep the air free from cigarette smoke or strong fumes. Do not inspect the throat with an object or use hot steam. Record a short description of the sound for the clinician if it comes and goes. A healthcare professional can decide whether medicine or nebuliser treatment is needed.

Get urgent help if the noisy inhalation continues while the child is resting, the ribs or neck pull in, the child struggles to speak or cry, swallowing causes drooling, lips look blue or grey, or the child becomes difficult to wake. Arrange medical advice for a worsening voice change, poor drinking, persistent fever, or a cough that is not settling. Breathing effort takes priority over the sound of the voice.

Ask whether the sound is hoarseness, a barking cough, or stridor; whether it occurs at rest; and how to monitor it overnight. Tell the clinician whether the child can drink, speak, sleep, and breathe comfortably between coughing episodes.
