What relief can and cannot do
Croup swelling affects the upper airway, producing a barking cough, hoarse voice, and sometimes stridor. Comfort measures can reduce distress, which may make breathing easier, but they do not remove airway swelling. A child’s breathing effort matters more than how dramatic the cough sounds. Check the child when settled: look for pulling at the ribs or neck, listen for harsh breathing in, and notice whether drinking and alertness are normal. Fever or a runny nose may accompany the infection.
Comfort measures
Hold or sit the child upright in a position they tolerate. Speak quietly and avoid unnecessary handling, because agitation can increase breathing difficulty. Offer frequent small sips if swallowing is comfortable. Keep the room free from tobacco smoke and strong irritants. Check the child regularly, including when resting, and follow only medicines prescribed or recommended for this episode. If symptoms are moderate or worsening, a clinician may assess the airway and consider treatment such as medicine delivered by a nebuliser.
When home relief is not enough
Call emergency services for stridor at rest, severe chest or neck indrawing, blue or grey lips, extreme tiredness, pauses in breathing, or drooling with difficulty swallowing. Seek urgent assessment if the child cannot drink, becomes unusually quiet or hard to wake, or deteriorates quickly. Contact a clinician when you are unsure whether the breathing is noisy at rest; do not wait for home comfort measures to fail completely.
