Croup RCH: what should you know?

RCH is often used as an abbreviation for a children’s hospital or its health information. The medical issue itself is croup: swelling in the upper airway that can cause a barking cough, hoarse voice, and noisy breathing in. A child may have cold symptoms or fever as well. The sound can be alarming, but severity is judged by breathing effort, colour, alertness, and ability to drink. Stridor only during crying is different from stridor that continues while the child is settled.

Stay with the child and use a calm voice. Keep them upright in a comfortable position and allow rest. Offer fluids in small amounts if swallowing is easy. Watch the chest and neck for skin pulling, and listen when the child is quiet. Avoid frightening examinations, forcing the child to lie flat, or giving medicines not prescribed for this episode. Seek emergency help for stridor at rest, marked chest indrawing, blue or grey colour, severe exhaustion, pauses in breathing, or difficulty swallowing saliva. A child who cannot drink, is becoming less alert, or is worsening despite calm positioning needs urgent assessment. If the cough is mild and breathing is comfortable, arrange clinical advice if symptoms persist, recur, or you are uncertain about the sound.
