What should you know about croup osmosis?

Croup osmosis is not a standard medical term; croup is a childhood airway illness that can cause a barking cough and noisy breathing. It commonly follows a viral infection and swelling around the voice box and upper windpipe creates the characteristic sound. The illness may be worse at night, and a child can also have a hoarse voice, runny nose, or fever.

Croup affects the upper airway rather than being a problem confined to the lungs. The swollen airway can make breathing in sound harsh or raspy; this noise is called stridor. A barking cough may become more noticeable when a child cries or becomes upset. Most children remain alert and able to drink, but symptoms can change, so watch breathing effort as well as the cough. A common cold can be the viral starting point, while the cough and fever are symptoms that may occur with the illness.

Keep the child calm and upright, because crying can make upper-airway narrowing sound worse. Offer small, frequent drinks if swallowing is comfortable, and check the child regularly, especially during sleep. Follow medicine instructions from a clinician or pharmacist; do not give cough medicines unless a healthcare professional has advised that product for the child. A clinician may assess the airway and prescribe treatment to reduce swelling. Nebuliser treatment may be considered in urgent care when breathing symptoms are significant, but it is not a home substitute for assessment.

Seek urgent medical help if the child has noisy breathing while resting, is working hard to breathe, has pulling in around the ribs or neck, cannot speak or cry normally, is drooling or struggling to swallow, or has blue, grey, or unusually pale lips or skin. Get immediate emergency help if the child becomes very sleepy, difficult to wake, floppy, or stops breathing normally. Arrange a child health consultation if the cough is worsening, fever persists, drinking falls, or you are unsure whether the breathing noise is stridor. Keep the child upright while waiting for help.

Ask whether the breathing noise is stridor and whether it occurs only during coughing or also while the child is resting. Ask which symptoms should prompt emergency care, how to manage fever and fluids, whether prescribed treatment is needed, and when the child can return to nursery or school. Tell the clinician when the symptoms began, whether they are worse at night, and whether the child has had similar episodes.
