Croup inflammation: why breathing sounds change

Short answer
Croup inflammation narrows the upper airway, creating a barking cough or noisy inhalation that must be judged by breathing effort.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How inflammation causes symptoms
The larynx and upper windpipe become swollen, often during or after a viral cold. Because this passage is relatively narrow in young children, swelling can make airflow noisier. The result may be hoarseness, a barking cough, or stridor when breathing in. Crying and agitation increase airflow demand and can make the sound more obvious. Fever and a runny nose may accompany croup, but their presence or absence does not show how narrow the airway is. A clinician assesses the child rather than judging severity by the cough alone.
Reducing strain while arranging care
Keep the child calm, hold them upright, and use a quiet voice. Offer small drinks if swallowing is easy. Remove smoke and other airway irritants from the room. Do not press a tongue depressor into the throat or use hot steam. Medical treatment may include medicine to settle airway swelling; severe symptoms can require observation and nebuliser treatment. Follow the exact instructions for any medicine already prescribed, and do not use someone else's inhaler or cough remedy. A child health consultation can clarify whether inflammation is improving.
Breathing signs that cannot wait
Get emergency help if noisy breathing continues while the child is calm, the ribs or neck pull in with each breath, the child becomes blue, grey, limp, or difficult to wake, or swallowing is so difficult that drooling develops. Seek prompt medical advice for increasing work of breathing, inability to drink, repeated vomiting, or a child who seems unusually exhausted. Keep observing the child even if the cough temporarily settles after rest.
Ask about the inflammation
Ask what examination findings show improvement, whether treatment is needed for airway swelling, how to monitor overnight, and what breathing sound should trigger emergency help.

Questions people ask
Does fever measure croup severity?
No. Breathing effort, alertness, swallowing, and the presence of noisy breathing at rest are more useful warning features.
Can inflammation make croup worse at night?
Symptoms can seem more noticeable at night; monitor calmly and seek help for breathing difficulty at rest.
