The short answer
Croup can make a child’s voice hoarse because swelling around the voice box changes how air moves through the upper airway. The same swelling may cause a harsh, barking cough and a rough or husky sound when the child cries or speaks. Croup commonly follows a viral upper-respiratory illness and is usually assessed by the child’s breathing, alertness, and ability to drink as well as by the sound of the voice.
What the hoarse voice means
A hoarse voice is a change in sound, not a measure of how severe the airway swelling is by itself. A child may sound raspy after coughing or crying, while a high-pitched, noisy breath can suggest narrowing higher in the airway. Listen when the child is calm, because distress and crying can make noisy breathing more noticeable. Watch for chest pulling in, fast or difficult breathing, drooling, or trouble swallowing.
Hoarseness can also occur with a common cold, irritation from coughing, or acid reflux, so the voice change alone does not identify its cause.
What to do at home
Keep the child calm and upright on a caregiver’s lap or in a comfortable sitting position. Crying can increase the effort of breathing. Offer frequent small drinks if swallowing is comfortable, and check the child regularly, including during sleep. Follow the clinician’s instructions if prescribed medicine for croup; do not give a child someone else’s medicine.
Avoid forcing steam inhalation or placing the child near hot water, because burns can occur and steam does not reliably treat the airway swelling. Avoid smoke and other airway irritants.
When to seek care
Get emergency help immediately if the child is struggling to breathe, has noisy breathing while resting, has blue or grey lips, cannot speak or cry normally because of breathlessness, is unusually sleepy or difficult to wake, or is drooling and unable to swallow. Keep the child upright and calm while help is arranged.
Arrange prompt medical assessment if the cough or hoarseness is worsening, drinking is reduced, the child has fewer wet nappies or trips to the toilet, or symptoms return after seeming to improve. A clinician can decide whether treatment is needed and check for another cause of airway symptoms.
