Croup dog: understanding a barking cough in children

Croup irritates and narrows the upper airway near the voice box. The resulting cough can sound harsh, hollow, or seal-like, and the voice may become hoarse. Some children also make a high-pitched sound when breathing in, called stridor. The cough can become more noticeable at night or after crying. A barking cough can occur with a viral illness such as a common cold, but the sound alone cannot show how severe the airway swelling is.

Stay close, keep your voice calm, and let the child choose a comfortable upright position. Offer sips of fluid when breathing is settled. Avoid upsetting the child, smoke, and hot steam. Watch whether noisy breathing occurs only during crying or also while the child is resting. Arrange a child-health consultation if you need help recognising stridor, if the cough is worsening, or if the child has other symptoms such as fever. A clinician may prescribe medicine that reduces airway swelling.

Call emergency services when the child is working hard to breathe, the skin pulls in between the ribs or above the breastbone, stridor persists at rest, lips look blue or grey, the child cannot drink, drools, becomes floppy, or is difficult to wake. Do not wait for the barking cough to disappear before seeking help. If breathing is comfortable between coughs, contact a clinician for tailored advice and continue observing closely.

Ask whether the sound is stridor or simply a cough, what breathing changes should trigger emergency care, whether a steroid is appropriate, and how to check the child overnight. Tell the clinician about fever, reduced urine, allergies, and any previous croup episodes.
