Croup differential diagnosis

Croup commonly causes a barking cough, hoarseness, and stridor from swelling near the upper airway, often following cold symptoms. A clinician listens for the timing of the noise, especially whether it occurs at rest, and checks breathing effort, colour, alertness, temperature, hydration, and ability to swallow.

The differential may include a common cold with cough, another upper-airway problem, a lower-airway illness, or an inhaled object. Sudden onset during eating or play, drooling, a muffled voice, or severe illness changes the urgency and needs immediate assessment rather than home comparison.

Record when the cough began and whether it followed choking, a cold, or exposure to an irritant. If safe, note whether noisy breathing is present while calm. Keep the child upright and settled, offer no forced food or drink when swallowing is difficult, and bring a list of medicines or previous airway problems.

Seek emergency help for severe breathing effort, blue or grey lips, pauses, collapse, confusion, drooling with inability to swallow, or rapidly worsening symptoms. Arrange urgent clinical review for stridor at rest, persistent chest or neck pulling, poor drinking, unusual sleepiness, or a sudden cough after possible choking. Do not delay care while trying to identify the exact label at home.
