Is croup dangerous?

Croup causes inflammation around the upper airway. A barking cough and hoarse voice may be accompanied by stridor, a harsh sound when breathing in. The airway is small in young children, so swelling can make each breath more effortful. Many episodes remain mild, but the severity can change over hours, including at night.

The cough itself is not the only measure of risk. A child who is alert, drinking, and breathing comfortably between coughs is different from one who is struggling, quiet from exhaustion, or unable to swallow.

Keep the child upright and calm, and avoid upsetting them unnecessarily. Offer fluids only if swallowing is comfortable. Do not use steam or place anything in the throat. If a clinician recommends medication or nebuliser treatment, follow the prescribed instructions and do not delay emergency help while trying home measures.

Call emergency services for severe breathing effort, blue or grey lips, pauses in breathing, inability to swallow saliva, drooling, confusion, collapse, or extreme drowsiness. Seek urgent assessment for stridor at rest, worsening chest or neck pulling, poor fluid intake, or symptoms that return after treatment. A child with a mild cough but normal breathing still needs reassessment if the pattern changes.

Ask what the child’s breathing sounds mean, whether an examination is needed today, and which local emergency number to use. Discuss any prescribed treatment, expected response, and the signs that mean it should not be repeated without advice.
