Croup cough without fever

Croup involves swelling near the voice box and upper airway, commonly during or after a viral infection. Fever may be absent or mild, so its absence does not confirm or exclude croup. A barking cough, hoarse voice, and a rough or high-pitched sound when breathing in are typical clues.

Look for the pattern rather than relying on temperature. Noisy breathing only when a child cries is less concerning than the same sound while resting. Other explanations for a cough include a common cold, allergic rhinitis, asthma, or acid reflux, so persistent or unusual symptoms deserve assessment.

Keep the child upright, quiet, and close to a familiar adult. Encourage small sips of fluid if swallowing is easy. Do not use hot steam, and do not force a throat examination. Check breathing when the child is calm, note whether the noise is present at rest, and observe urine output and alertness as signs of hydration.

Get emergency help for blue or grey lips, severe chest or neck pulling, pauses in breathing, drooling with inability to swallow, marked drowsiness, or rapidly worsening breathing. Seek prompt medical advice if stridor is present at rest, drinking is difficult, the cough is escalating, or the child seems exhausted even without fever. A mild cough with comfortable breathing may be monitored with clear safety advice.

Ask how to recognise stridor, how often to check breathing overnight, and which symptoms require emergency help. Tell the clinician whether the child is drinking, urinating, sleeping, and breathing comfortably when calm.
