What the cough can mean
Croup is swelling around the voice box and upper windpipe, often after a viral cold. The cough may sound barking or harsh and can be worse at night. A hoarse voice and a high-pitched noise when breathing in can occur. The sound is more concerning when it continues while the child is calm and resting, rather than only during crying.
Watch the child as a whole: colour, alertness, ability to swallow, and the effort needed for each breath matter more than the cough sound alone. A cough can also have other causes, including a common cold, asthma, or acute bronchitis.
What to do now
Keep the child upright and calm; crying can make upper-airway noise and breathing effort worse. Offer small, frequent sips if swallowing is comfortable. Avoid forcing food, examining the throat with an object, or using steam because hot vapour can burn. Note when the noisy breathing occurs, whether it changes at rest, and how much the child is drinking.
When to seek care
Seek emergency help immediately for struggling breaths, ribs or skin pulling in with breathing, blue or grey lips, pauses in breathing, drooling with difficulty swallowing, unusual sleepiness, or inability to speak or cry normally. Arrange prompt clinical assessment if the cough is worsening, noisy breathing appears at rest, fluids are falling, or symptoms return after improving. A child who is breathing comfortably but has a mild barking cough can still benefit from same-day advice.
