How is croup cough diagnosed?

A clinician listens to the cough and asks when it began, whether it worsens at night, and whether the child has fever, a hoarse voice or noisy breathing. They assess breathing effort, alertness, colour, hydration and whether the child can swallow. Watching the child while calm can be more informative than hearing a cough after prolonged crying.

Many cases do not need a throat swab, blood test or imaging because the pattern is recognisable and upsetting the child can worsen airway noise. The clinician still considers alternatives such as a common cold, bronchiolitis, asthma, an inhaled object or another cause of upper-airway obstruction.

Write down the time symptoms started and whether the cough is barking, hoarse or accompanied by a high-pitched noise when breathing in. Note breathing while resting, fluid intake, urine output, fever, possible choking and contact with illness. If safe, a short recording of the cough may help when the sound is intermittent, but never delay care to make one.

Keep the child calm and upright during travel or while waiting for advice. Avoid smoke, steam and unprescribed cough medicines. A clinician may recommend observation, medicine or further assessment according to the breathing findings.

Seek emergency help if the child struggles to breathe, has noisy breathing while resting, pulls in at the ribs or neck, develops blue or grey lips, drools or cannot swallow, becomes very sleepy, or cannot drink. These signs concern breathing and airway function, regardless of what caused the cough.

Ask what findings support croup, which alternative causes were considered, whether any medicine is needed, and what breathing changes should trigger emergency help. Clarify how to monitor overnight and when the child can return to normal activities.
