Croup cough in a 4-month-old

Croup is swelling around the upper airway, often after a viral infection. In a young infant, even modest swelling can affect airflow and make feeding tiring. A barking cough, hoarse cry, or harsh sound while breathing in may occur, but these signs can overlap with bronchiolitis, a common cold, reflux, or another respiratory problem. The cough sound by itself cannot establish the diagnosis.

Check the infant when calm: listen for noisy breathing, watch the ribs and neck for pulling in, and notice whether the baby wakes, responds, and takes usual feeds.

Hold the baby upright, keep the surroundings quiet, and offer smaller feeds more often only if breathing and swallowing are comfortable. Do not give honey, over-the-counter cough remedies, or steam. Keep the nose clear with gentle measures recommended for infants. Record a brief description of the cough and whether the sound appears at rest, during crying, or during feeds; this can help the clinician assess the pattern.

Call emergency services if the baby has pauses in breathing, blue or grey lips, severe chest indrawing, grunting, limpness, difficulty waking, choking, or cannot coordinate breathing with feeding. Seek urgent assessment for persistent noisy breathing at rest, markedly reduced feeds, repeated vomiting, or fewer wet nappies. Do not wait for the cough to become louder.
