What should you know about croup and newborns?

Croup causes swelling around the upper airway and is recognized by a barking cough or a harsh sound when breathing in. In a newborn, however, cough, noisy breathing, poor feeding, changes in colour, or unusual sleepiness can have several causes. A blocked nose alone can make feeding harder, while lower-airway or other infections may also affect breathing.

Because newborns have small airways and limited reserves, their condition can change quickly. Do not try to confirm croup at home or rely on the cough sound alone; an examination is the safest way to judge breathing and hydration.

Keep the baby upright while awake, keep the environment smoke-free, and offer usual feeds only if breathing and swallowing are comfortable. Note the timing of symptoms, temperature measurement, wet nappies, feeding amounts, and any change in breathing to tell the clinician.

Contact a child health service promptly for assessment. Do not give cough remedies, honey, or medicines intended for older children unless a clinician specifically advises them. Nebuliser treatment is used only when a clinician decides it is appropriate.

Seek emergency help immediately for pauses in breathing, struggling or grunting breaths, ribs pulling in, blue or grey lips, inability to feed, repeated vomiting with weakness, a floppy or difficult-to-wake baby, or a sudden worsening. Any newborn with fever should receive prompt medical advice, even if the cough seems mild.

Ask what is causing the breathing noise, whether the baby needs observation or testing, how to support feeds safely, how to check hydration, and which symptoms mean you should return without delay.
