Cribriform DCIS on a Pathology Report

DCIS means abnormal cells remain confined by the duct's basement membrane and have not invaded surrounding breast tissue. Cribriform describes their microscopic architecture; it does not by itself determine the full treatment plan.

The report may describe nuclear grade, cell death called necrosis, the measured extent and whether hormone receptors are present. After surgery, it also records the distance between DCIS and the cut tissue edges, known as margins. These details are considered together rather than treating the cribriform pattern as a stand-alone forecast.

A core biopsy samples only part of an abnormal area. The surgical specimen can provide a broader view and may occasionally identify another pattern or an invasive focus not represented in the initial cores. Radiology and pathology findings are therefore checked for agreement.

The breast team matches the pathology with the mammogram or other imaging to estimate the area involved. Breast-conserving surgery may be suitable when the area can be removed with acceptable margins and breast shape. Mastectomy may be discussed for more extensive disease or when clear margins cannot be achieved with conservation.

Radiotherapy may follow breast-conserving surgery to reduce recurrence within the treated breast. If the DCIS has hormone receptors, hormone therapy may be offered in selected circumstances. Decisions reflect the complete report, operation performed, health priorities and personal preferences.

DCIS is often found on breast imaging rather than through symptoms. Arrange assessment for a new breast lump, bloody nipple discharge, nipple change or new skin dimpling; these findings have several possible causes and need examination rather than self-diagnosis.

After biopsy or surgery, contact the care team for increasing redness, warmth, swelling, drainage, fever, pain that is not controlled or bleeding that does not settle with the advised pressure. Seek emergency help for severe breathing difficulty, fainting or rapidly worsening swelling.

Ask which nuclear grade was found, whether necrosis or invasion is present, how large the involved area appears, and whether imaging matches the biopsy. After an operation, ask whether the margins are clear, whether another procedure is advised, and how receptor findings affect radiotherapy or hormone-therapy discussions.
