What should you know about a cough X-ray?

The test uses a small amount of radiation to create images of the chest. It may be considered when a cough persists, breathing symptoms are present, or a clinician needs to look for an infection or another chest problem. A normal image does not rule out causes such as asthma, reflux, allergies, or irritation of the airways.

An X-ray can show features such as a patchy area in the lung, fluid, a collapsed area, or an enlarged heart. These findings do not provide a diagnosis on their own; the clinician matches them with your cough, temperature, breathing, medicines, smoking history, and other health conditions.

If the image is clear, the cough may still come from the upper airways, sensitive airways, acid reflux, or a recent viral illness. Depending on the findings, your clinician may recommend observation, another test, or treatment. Some airway conditions do not appear clearly on a standard X-ray.

Tell the imaging team if you are pregnant or might be pregnant, and mention any relevant medical devices or difficulty standing. You can usually eat and drink normally unless your healthcare team gives different instructions. Wear clothing that allows easy access to the chest and follow instructions to hold still or briefly hold your breath.

Keep track of when the cough began, whether it brings up mucus, and what makes it worse. Take prescribed medicines as directed, and avoid cigarette smoke and other airway irritants. Do not start antibiotics just because an X-ray was requested; treatment depends on the overall assessment.

Seek urgent medical help for severe or rapidly worsening breathlessness, blue or grey lips, new confusion, fainting, severe chest pain, or coughing up more than a small streak of blood. These symptoms need prompt assessment regardless of whether an X-ray has been arranged or was normal.

Arrange a clinical review if the cough is persistent, repeatedly returns, disturbs sleep, or comes with fever, wheezing, unexplained weight change, night sweats, or increasing tiredness. Children, older adults, pregnant people, and people with lung or immune conditions may need earlier advice.

Ask what the X-ray is intended to check, whether your symptoms suggest another test, and when you should expect the result. You can also ask which warning signs should prompt urgent help and whether any current medicine, allergy, reflux symptom, or exposure could be contributing to the cough.
