Cough without fever: what to know

Fever is only one possible feature of illness and may be absent when the airways are irritated rather than causing a strong whole-body response. A common cold or acute bronchitis can produce cough without fever, especially as other symptoms settle. Allergic rhinitis may cause throat clearing and cough, while asthma can cause cough with chest tightness or wheeze. Acid reflux may trigger cough after meals or when lying down. A cough that continues beyond the expected recovery period is described as chronic cough and deserves review.

Note whether the cough is dry or brings up mucus, its timing, exposures, sleep effect and any breathlessness. These clues are more useful than fever alone.

Drink regularly, avoid smoke and ventilate rooms around dust, sprays or fragrances. Honey in a warm drink may soothe the throat for adults and children over one year; never give honey to an infant under one year. Consider a pharmacist’s advice before using cough medicines, particularly for children, pregnancy or long-term conditions. If reflux seems connected, avoid lying down soon after eating and discuss recurring symptoms with a clinician.

Seek urgent help for severe breathing difficulty, chest pain, coughing blood, confusion, fainting or blue lips. Arrange a medical review when the cough is worsening, disrupts sleep, returns repeatedly, follows a significant exposure, or occurs with weight loss, night sweats or persistent breathlessness. A clinician can examine you and decide whether the chest, nose, reflux or medicines need attention.
