Cough with wheeze at the end: what to know

Wheezing is a musical or whistling sound caused by air moving through narrowed air passages. It may follow a cold or acute bronchitis, occur with asthma, or be triggered by allergens such as dust or animals. Acid reflux can irritate the throat and contribute to cough. A repeated cough with wheeze, breathlessness or thick mucus needs a clinician to consider conditions such as chronic cough or bronchiectasis. In infants, bronchiolitis can cause cough and noisy breathing.

Record when the sound appears: during exercise, at night, around a trigger, or only after repeated coughing. A recording can help if the sound is absent during the appointment.

Move away from smoke, strong fragrances and known irritants. Sip fluids and rest your voice. If you already have a prescribed reliever inhaler, follow your personal action plan rather than borrowing someone else’s medicine. Do not use antibiotics or inhalers without appropriate advice. Arrange an assessment if wheeze keeps returning, follows a new exposure, disturbs sleep, or limits normal activity.

Seek emergency help for severe breathlessness, blue or grey lips, inability to speak in full sentences, confusion, fainting, or sudden wheeze after choking or an allergic exposure. Prompt medical review is also needed for chest pain, rapidly worsening breathing, or a child who is unusually drowsy or struggling to feed. A clinician can listen to your chest and check oxygenation and the pattern of symptoms.
