Cough with phlegm and wheezing: what to know

Phlegm is mucus brought up from the airways. Wheezing is a high-pitched sound caused when air moves through narrowed passages. Acute bronchitis can irritate the larger airways and produce cough with mucus. Asthma can cause variable narrowing, often with chest tightness or breathlessness. COPD may cause ongoing cough and phlegm, especially in people with a smoking or occupational exposure history. Allergic rhinitis can add post-nasal drainage, while bronchiolitis is more relevant in young children. A symptom alone cannot identify the cause.

Rest, drink fluids if you can do so comfortably, and avoid smoke, vaping, dust and strong fragrances. Note the colour and amount of phlegm, triggers for wheezing, temperature, and whether symptoms are changing. Use a prescribed reliever inhaler exactly as directed; do not borrow someone else’s inhaler or start antibiotics without clinical advice. A clinician may listen to your chest and decide whether inhaled treatment, such as nebuliser treatment, is appropriate.

Seek emergency help for severe breathlessness, blue or grey lips, confusion, fainting, or difficulty speaking because of breathing. Arrange urgent assessment for rapidly worsening wheeze, chest pain, coughing blood, or a high fever with marked weakness. Contact a clinician if the cough persists, keeps returning, disrupts sleep, or you have asthma, COPD, heart disease, or a weakened immune system.
