Cough, wheezing and congestion: what to know

Congestion may mean a blocked nose, mucus in the throat or chest phlegm. Wheezing is a musical breathing sound caused by narrowed airways, while coughing helps clear irritation or mucus. A viral infection or Acute Bronchitis can produce this cluster; Allergic Rhinitis can add nasal blockage and throat drainage.

Asthma can present with episodes of cough and wheeze, and COPD is another possible cause in some adults. Bronchiolitis is assessed differently in infants and young children. Symptoms alone cannot identify the cause, particularly when breathing becomes difficult.

Rest, drink fluids, avoid smoke and monitor whether breathing is becoming easier or harder. Saline nasal measures may ease nasal congestion for some people. Use prescribed inhalers exactly as directed, and do not start antibiotics or a nebuliser without clinical advice.

Record fever, phlegm, triggers, activity limits and how quickly symptoms developed. Arrange an assessment if wheezing is new, symptoms are recurring, or congestion and cough are not improving. The clinician may listen to your chest and decide whether breathing tests or other treatment is needed.

Get emergency help for severe breathlessness, blue or grey lips, inability to speak in full sentences, fainting, confusion, or sudden wheezing with facial or throat swelling. Seek prompt medical advice for chest pain, coughing blood, high fever, or a clear deterioration after initial improvement.

Ask whether the congestion is nasal or chest-related, what may be triggering the wheeze, and how to use any prescribed inhaler. Ask what signs mean you should seek urgent care and when a follow-up is needed.
