What the sound can mean

Croup commonly causes a seal-like bark, a hoarse voice, and a rough sound when breathing in. It often follows cold symptoms and may sound worse at night. The sound can come from narrowed tissue near the voice box rather than mucus deep in the chest. Bronchiolitis affects smaller airways and may instead cause wheeze, fast breathing, or feeding difficulty in young children. A clinician may listen to the breathing and look at the child's effort, colour, alertness, and ability to drink.

The sound alone cannot confirm the cause. A sudden cough after food or a small object may signal choking, which needs immediate attention. A cough that continues after the noisy phase may also need assessment for asthma or another airway problem.

Do not use steam from boiling water because it can cause burns, and do not force food or drink if breathing is difficult. Note whether the noisy breathing occurs only while coughing or also when the child is settled, and record when symptoms started for the clinician.

Get emergency help if the child is struggling to breathe, the skin between the ribs or at the neck is pulling in, lips look blue or grey, the child is unusually sleepy, or they cannot speak, cry, swallow, or drink normally. Noisy breathing while resting needs urgent assessment, especially if it is worsening. Seek prompt medical advice for repeated episodes, a high fever with marked illness, drooling with difficulty swallowing, or a cough that began suddenly during eating or play. If symptoms are mild but do not settle, arrange a clinical review.
