COPD and heart failure symptoms: what to know

COPD and heart failure can both cause breathlessness, cough, tiredness and reduced exercise tolerance, so new or worsening symptoms need clinical assessment. COPD mainly affects airflow through the lungs, while heart failure affects how effectively the heart moves blood around the body. Either condition can make ordinary activity feel harder, and they can occur together. Symptoms alone cannot reliably identify the cause.

COPD often causes an ongoing cough, phlegm, wheezing or chest tightness, with breathing trouble that may flare during an infection or after exposure to smoke and other irritants. Heart failure may cause breathlessness when lying flat or waking at night short of breath. Fluid retention can lead to swollen ankles, legs or abdomen and quick changes in how tight shoes or clothing feel. Fatigue may occur with either condition because breathing and circulation demand more effort. A racing, fluttering or irregular heartbeat can have several causes, including atrial fibrillation, and should be assessed rather than attributed to COPD.

These patterns are clues, not a diagnosis. A clinician may review your medical history, listen to your chest, check oxygen levels and examine swelling, then arrange tests suited to your symptoms.

Arrange a medical review if breathlessness, cough, swelling, fatigue or palpitations are new, becoming more frequent, or limiting your usual activities. Keep taking prescribed medicines as directed, and do not change inhalers, diuretics or heart medicines without advice. Note when symptoms occur, whether they are worse with activity or lying down, any change in phlegm, and whether swelling is affecting both legs. Bring this information and a list of medicines to the appointment. A chronic disease review can help coordinate monitoring when COPD or heart failure is already diagnosed.

Seek emergency help immediately for severe or rapidly worsening breathlessness, inability to speak in full sentences, blue or grey lips, fainting, new confusion, or chest pressure with sweating or nausea. Urgent assessment is also needed for sudden breathlessness at rest, coughing up pink or bloody foam, or a fast or irregular heartbeat with dizziness or chest pain. Sit upright while waiting for help, use a prescribed reliever inhaler according to your action plan, and do not drive yourself if you feel faint or severely short of breath.

You can also ask which warning signs should prompt urgent help, how to use each inhaler, whether your medicines could worsen breathlessness or swelling, and which checks will monitor your lungs and heart.
