COPD and congestive heart failure: what to know

COPD narrows and damages airways, while heart failure means the heart is not pumping effectively enough for the body's needs. A person can have both conditions, and each can make exertion feel harder.

Heart failure may cause swollen ankles, unusual tiredness, a cough, or breathlessness that is worse when lying down. COPD may bring wheezing, chest tightness, increased phlegm, or a change in usual breathing. These patterns overlap, so a symptom alone cannot identify the cause.

Arrange a clinical review if breathlessness is new, gradually worsening, or different from your usual COPD pattern. Take a list of inhalers, tablets, allergies, and recent changes in sputum or exercise ability.

Assessment may include listening to the chest, checking oxygen levels and heart rhythm, reviewing blood pressure, and arranging breathing or heart tests when needed. Use prescribed medicines exactly as directed; do not increase diuretics or rescue inhalers without advice.

Seek emergency help for severe or rapidly worsening breathlessness, blue or grey lips, fainting, new confusion, severe chest pressure, or an inability to speak in full sentences. Sudden palpitations with weakness or breathlessness also need urgent assessment.

Contact a clinician promptly if ankle swelling increases, you gain fluid weight quickly, cough up pink or frothy sputum, or your usual inhaler is not relieving a change in symptoms.
