Convulsion without fever: what should you know?

A seizure may cause stiffening, rhythmic jerking, staring, loss of awareness, or confusion afterwards. No fever does not identify the cause or rule out a serious problem. Possible triggers include a blood-sugar drop, alcohol withdrawal, a medication effect, sleep loss, head injury, or an underlying seizure disorder. A clinician may ask about what happened before, during, and after the event and may consider blood tests, an electrical brain test, or imaging based on the history.

Time the event and remove nearby hazards. Cushion the head, loosen clothing around the neck, and do not hold the person down or place anything between the teeth. Do not give water, tablets, or food while awareness is reduced. Once movements stop, turn the person onto their side, check breathing, and stay with them while they recover. Record observations for the clinician, including missed medicines, alcohol changes, new drugs, sleep deprivation, and possible injury.

Call emergency services for a first convulsion, an event that lasts several minutes, repeated seizures without recovery, breathing difficulty, serious injury, or failure to regain awareness. Emergency assessment is also needed for new weakness, trouble speaking, severe headache, persistent vomiting, or confusion that is worsening. Do not drive yourself after a suspected seizure; arrange safe transport and follow local medical advice about driving and work at heights or around machinery.

Ask which triggers are plausible, whether your medicines or alcohol pattern could contribute, what tests are appropriate, and how to reduce injury risk while the cause is being clarified. Ask for written instructions covering recurrence, bathing, driving, and rescue treatment.
