Constipation and spotting in early pregnancy

Early pregnancy can bring slower bowel movement, harder stools, bloating, and straining. Spotting means blood from the vagina and may have several causes, including changes around the cervix; constipation does not explain every episode of bleeding. The two symptoms may happen at the same time without one causing the other. Note the colour, amount, timing, clots, pain, and whether bleeding follows sex or straining, because these details help a clinician assess you.

Use gentle measures for constipation: sip fluids regularly, add fibre slowly, walk if comfortable, and avoid forceful straining. Do not take a laxative or herbal remedy without pregnancy-specific advice. Contact your maternity service or pregnancy clinician about any spotting, even when it is light, and follow their instructions about observation, examination, or testing. Keep a record of bleeding and bowel symptoms, and use a pad rather than relying on guesswork about the amount.

Seek urgent medical care for heavy or increasing bleeding, severe or one-sided abdominal pain, shoulder-tip pain, dizziness, fainting, fever, or passing tissue. Urgent review is also needed if vomiting prevents fluids or your abdomen becomes very swollen and you cannot pass gas. If spotting continues, returns, or is accompanied by cramps, contact your maternity team promptly even if constipation is improving.
