Congestive heart failure wheezing
Updated

Short answer
Congestive heart failure can cause wheezing when fluid congestion narrows the small airways, but wheezing also has other possible causes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible pattern
- Wheezing may accompany breathlessness, cough, or difficulty lying flat.
- Other causes
- Asthma, infection, allergies, and other lung conditions can also wheeze.
What should you know about congestive heart failure wheezing?
Congestive heart failure can cause wheezing when fluid congestion narrows the small airways, but wheezing also has other possible causes. The sound may occur with breathlessness, cough, or difficulty lying flat. Asthma, infection, allergies, and other lung problems can also wheeze, so the sound itself does not establish heart failure.
How heart failure may cause a wheeze
When pressure builds in blood vessels around the lungs, fluid can affect the airways and make breathing noisy. You may also notice rapid breathing, reduced exercise tolerance, fatigue, or swollen ankles. A clinician will consider the timing, chest findings, existing lung conditions, medicines, and heart history. Atrial Fibrillation and Hypertension may be relevant to the wider assessment.
What to do when wheezing starts
Arrange medical advice for new wheezing, repeated episodes, or wheezing with a change in usual heart-failure symptoms. Sit upright, rest, and use only inhalers or heart medicines prescribed for you. Record whether symptoms follow activity, lying down, an infection, or a medicine change, and note cough, swelling, palpitations, and breathlessness. Do not borrow another person’s inhaler.
When wheezing is an emergency
Call emergency services for severe breathing difficulty, inability to speak normally, blue or grey lips, fainting, new confusion, chest pressure, or rapidly worsening wheeze. Urgent assessment is also needed when wheezing comes with breathlessness at rest or sudden swelling. Remain upright while waiting and follow instructions from emergency services.
Questions for your doctor
Ask whether the wheeze sounds more consistent with fluid congestion, asthma, infection, or another cause; which tests are needed; and what symptoms should trigger emergency help. Ask whether a Chronic Disease Review can include a personalised breathing action plan.
