Is congestive heart failure hereditary?

Heart failure means the heart is not pumping or filling well enough for the body’s needs. It can follow coronary artery disease, long-standing high blood pressure, valve disease, abnormal heart rhythms or damage to the heart muscle. Some cardiomyopathies and rhythm conditions can run in families, while many other causes are acquired. A relative with heart failure therefore does not establish the cause in another family member.

Tell a clinician if a close relative had unexplained heart failure, a heart muscle disorder, fainting, serious rhythm problems or sudden cardiac death at a young age. The family pattern, age at diagnosis and known test results help determine whether family assessment is useful. Symptoms such as breathlessness, unusual tiredness, ankle swelling or a persistent cough need their own evaluation.

Write down which relatives were affected, their age when symptoms began, and whether they were diagnosed with cardiomyopathy, atrial fibrillation or hypertension. Arrange a clinical assessment if you have symptoms or a strong family pattern. Assessment may include an examination, blood tests, an electrocardiogram and an echocardiogram; the clinician chooses tests based on your history.

Seek emergency care for severe or rapidly worsening breathlessness, chest pressure, fainting, blue or grey lips, new confusion, or palpitations with collapse. Contact a clinician promptly for new swelling in both ankles, a cough that is worse when lying down, or a clear change in exercise tolerance. These symptoms can have several causes and should not be attributed to family history alone.
