What should you know about congestive heart failure and restless legs?

Restless legs syndrome causes an urge to move the legs with crawling, pulling or uncomfortable sensations, usually during rest and often in the evening; movement may briefly help. Heart failure more commonly causes breathlessness, fatigue, cough, palpitations or swollen ankles because the heart cannot circulate blood efficiently. Swelling and heaviness are not the same as the urge-to-move pattern. Kidney disease, low iron, nerve problems, medicines and poor sleep can also contribute to restless legs. Atrial fibrillation and hypertension may coexist with heart failure and affect its management.

Keep a record of when the sensations occur, whether movement helps, and any change in ankle swelling or breathing. Continue prescribed heart medicines and do not stop a diuretic or other treatment without advice. Reduce late caffeine, keep regular sleep times and use gentle movement or stretching if comfortable. Arrange a chronic disease review to discuss symptoms, medicines and possible blood tests, including checks relevant to iron or kidney function. Avoid starting iron or supplements without professional guidance.

Call emergency services for severe or sudden breathlessness, chest pain, fainting, blue lips, new confusion or coughing pink frothy fluid. Contact your heart-care team promptly for rapidly increasing ankle swelling, a clear weight increase over a short period, worsening breathlessness when lying down or new sustained palpitations. Seek routine assessment when restless legs repeatedly disrupt sleep, spread to the arms or begin after a medicine change.
